I get a ton of warning in game about pings, but never a kick for high ping.

In my ping.plugin I have these lines

   <settings name="settings">
      <set name="interval">30</set>
      <set name="max_ping">400</set>
      <set name="max_ping_duration">90</set>
   </settings>
   <settings name="commands">
      <!-- The command level for !ci -->
      <set name="ci">20</set>
   </settings>
</configuration>

Do I need to set a number of warning and a kick line somewhere?

pingwatcher does not kick, it just notifies so admin can act upon.
Maybe you're looking for pingkicker

Just a heads up, installed on my San Fran server, it was full, after install went to eight, lol.
Ping kick was set at 100 really should be alot higher for bc2 and the ping reporting in game.
I set mine for 400 for now.
