We currently have a critical problem with our servers. Lately it seems that Punkbuster will just stop returning GUID's for players causing B3 to fall back onto regular GUID's and entering our members again. It has caused me to restore the database a few times to undo this damage.

Has anyone else encountered this? Solutions?

This is still the case, I am having to drop users from the database on a daily basis to restore their access. I do not know what is going on but I need help.

You're using an older version of the bot. Please update to v1.3.3 or wait for v1.4.0 that comes out next week and try again.

I also see a lot of connection refused messages in your startup, make sure all settings are correct.

Ill update however, I do not believe that to be the root cause of the issue.

The connection refused, I saw and believe it to be because b3 is loading faster than MYSQL on a reboot. I placed B3's service on a delayed start, was hoping that would clear up that issue.

I didn't update to 1.3.3 as it states that it is BFBC2 related making it pointless for COD5.

Well, core  changes where made. So I would wait until next week when we release version 1.4.0 and you report back after updating.
