Yes, a bugged sv_maprotation can cause this. Please check all the rotations from rotationmanager.xml for similar problems.

I've taken a look at game.py, this error is caused by the map end not followed by a map start. Can be safely ignored, the only problem it may cause is a bugged status.xml (not sure if really causes), but it's not a problem when the server is a walking corpse, right?

P.S. I meant the sv_maprotationcurrent from the server (obtained using any rcon tool). If there's a bugged map in its value, we've found the problem.

I just installed this on my MW2 servers and I am not sure if this is a bug or a fluke, but we were playing one map and the next map was the same map. I manually rotated the map, but I was wondering if anyone else experienced this?

Can you quote the Rotationmanager Plugin related parts of the b3.log?
I did some changes in it for Black Ops, maybe I see the problem with MW2 there.

Can you provide the new map names so I could update the plugin? Don't forget that you need to edit your xml file as well, this line:

works great on mw2 Servers, but when the Server crashed after restart the Maprotation not works always

I guess you just did wrong copy and paste. Kill out the "maps: " in the xml and it will work.
